Most recent 2 Years W-2 forms
Most recent 2 years signed IRS Tax returns with all schedules including copies of K-1's
1 months pay stubs
Most recent 2 months bank statements, all pages
Most recent retirement account statements, all pages
Copy of your DL and SS Card
Name, phone number of your insurance agent

If you are refinancing an existing mortgage:  

Copy of the closing papers when you purchased the home
Most recent mortgage statement - showing the outstanding balance
Copy of your existing survey

If applicable:

Please provide a copy of your divorce decrees, child support agreements, Bankruptcy papers and discharge.
